NHMS Secures a Planning Grant to Research the Impact of Chronic Disease

The New Hampshire Medical Society (NHMS) recently received a planning grant from the New Hampshire Department of Health and Human Services. The purpose of the grant is to explore ways the Medical Society can help expand use of chronic disease resources and improve access and care for treatment of diabetes, heart disease, cancer, etc. NHMS is looking forward to working with members and healthcare partners to conduct an industry scan and a statewide survey. Through this work, we will identify key gaps and barriers in care delivery that can guide future strategies to enhance patient outcomes and better support healthcare providers.
The planning grant will be effective from late January through March 2025. The NHMS is very interested in helping raise awareness and increase access to innovative resources for patients with chronic diseases. This is an opportunity to better understand the barriers that both physicians and patients face and to advance public health initiatives in 2025.
